ABSTRACT

Resumo Introdução: Este artigo aborda a ocorrência de agravos à saúde em virtude de ferimentos na face por arma de fogo, entre os policiais militares, na Região Metropolitana do Estado do Rio de Janeiro, operados pela Clínica de Cirurgia e Traumatologia Bucomaxilofacial do Hospital Central da Polícia Militar. Objetivo: Identificar o perfil dos pacientes operados em decorrência de ferimentos na face por arma de fogo, a distribuição anatômica das fraturas maxilofaciais, as sequelas e complicações encontradas, as especialidades em saúde envolvidas na reabilitação desses pacientes, além de discutir sobre as repercussões sociais, emocionais e relativas ao desempenho do trabalho entre esses sujeitos. Método: Foi feito um estudo epidemiológico retrospectivo a partir de dados secundários referentes aos policiais militares operados no Hospital Central da Polícia Militar em decorrência de ferimentos por arma de fogo em face, de junho de 2003 a dezembro de 2017. Resultado: Durante o período estudado foram feitas 778 cirurgias em centro cirúrgico pelo serviço de Clínica de Cirurgia e Traumatologia Bucomaxilofacial no Hospital Central da Polícia Militar, 186 em decorrência de ferimentos por arma de fogo (23,9%). Todos os pacientes eram do sexo masculino e com média de 34,7 anos. A perda de segmento ósseo foi a sequela mais encontrada. O comprometimento estético facial e os relatos de insônia foram as repercussões tardias de impacto na saúde e no convívio social mais encontradas. Sobre as repercussões laborais do ferimento sofrido, o tempo médio de afastamento por licença de saúde para tratamento dos ferimentos maxilofaciais foi de 11,7 meses. Conclusão: O tratamento de pacientes vítimas de ferimentos por arma de fogo em face demanda múltiplas intervenções cirúrgicas e o envolvimento de diferentes especialidades da saúde para sua reabilitação. São necessários mais estudos que analisem qualitativamente o impacto desse tipo de traumatismo em face para a vida dos pacientes e seus desdobramentos sociais.

ABSTRACT

Objetivo: Analizar y describir los requerimientos de atención quirúrgica bucomaxilofacial de urgencia en el Servicio de Urgencias Odontológicas y Orientación de Pacientes de la Facultad de Odontología de la Universidad de Buenos Aires durante el aislamiento social preventivo y obligatorio por la pandemia de COVID-19. Materiales y métodos: Se realizó un estudio observacional descriptivo de análisis retrospectivo durante el período de aislamiento social preventivo y obligatorio inicial de 93 días, comprendido entre el 20 de marzo y el 20 de junio de 2020. Se evaluó la totalidad de historias clínicas de los pacientes que acudieron al Servicio de Urgencias Odontológicas y Orientación de Pacientes de la Facultad de Odontología de la Universidad de Buenos Aires y se caracterizó a aquellos que requerían distintos tipos de tratamiento quirúrgico bucal y maxilofacial. Resultados: Durante el período estudiado, se atendieron 4564 pacientes, de los cuales 1337 (29,3%) requirieron tratamientos quirúrgicos como terapéutica para la resolución de la urgencia. De estos, el 93,2% fueron exodoncias de piezas erupcionadas o retenidas; el 1,7%, biopsias quirúrgicas; el 2,4%, tratamientos agudos de infecciones que involucran espacios anatómicos vecinos; el 0,8%, resolución de traumatismos en los maxilares, y el 1,9%, tratamientos de complicaciones posquirúrgicas. Conclusión: Los resultados ponen de relieve la necesidad de disponer de servicios de guardia odontológica en el ámbito del AMBA que cuenten con recursos humanos calificados y entrenados para resolver urgencias de tipo quirúrgico


Aim: The objective of this study was to analyze and describe the requirements for the emergency care of oral and maxillofacial surgical treatment in the emergency dental department of the School of Dentistry of the University of Buenos Aires during the Preventive and Mandatory Social Isolation. Materials and methods: We conducted a retrospective analysis of a descriptive observational study during the initial period of 93 days of Preventive and Compulsory Social Isolation, from March 20, 2020 to June 20, 2020. All the medical records of the patients who attended the emergency dental department of the School of Dentistry of the University of Buenos Aires were evaluated and those who required different types of oral and maxillofacial surgical treatment were characterized. Results: A total of 4564 patients were attended during the period studied, of which 1337 (29.3%) required surgical treatment as a therapy for the resolution of their emergencies. Of these, 93.2% were exodontia of erupted or retained teeth, 1.7% surgical biopsies, 2.4% acute treatment of infections involving neighboring anatomical spaces, 0.8% resolution of maxillary alveolar trauma and 1.9% treatment of post-surgical complications. Conclusion: The results highlight the need to have dental emergency services in the AMBA area that have qualified and trained human resources to solve the surgical type emergencies that may arise (AU)

Resumen Introducción: El trauma maxilofacial (TMF) constituye un problema de relevancia en los centros hospitalarios de alta complejidad. Objetivo: Evidenciar y actualizar la epidemiología del trauma maxilofacial de resolución quirúrgica bajo anestesia general, en pabellón central tratado en el Hospital de Urgencia Asistencia Pública durante el período comprendido entre diciembre de 2014 a diciembre de 2017 y compararla con la realidad epidemiológica de otros centros hospitalarios de alta complejidad en Chile y en el extranjero. Materiales y Método: Se realizó un estudio retrospectivo en el que se analizaron los protocolos operatorios de los pacientes intervenidos quirúrgicamente por trauma maxilofacial, en pabellón central, bajo anestesia general, en un período de 37 meses. Las variables analizadas fueron diagnóstico, edad, género, tercio facial y zona anatómica afectada. Resultados: Se realizaron 127 procedimientos quirúrgicos, para resolver 259 fracturas. La edad promedio de los pacientes intervenidos fue de 33 años, con predominio del género masculino (85,8%); el tercio inferior fue el más afectado (77,2%), específicamente en la zona parasinfisiaria mandibular (22,4%). Discusión: Los resultados expuestos concuerdan con estudios realizados internacionalmente, pero tiene discordancia en relación a la zona anatómica más afectada al comparalo con los otros centros que analizaron su epidemiología. Conclusión: Resulta necesario continuar la investigación epidemiológica de estas patologías y unificar criterios de diagnóstico y tratamiento para poder establecer políticas de prevención y tratamiento eficientes en relación a cada servicio y en conjunto a nivel de salud pública.


Introduction: Maxillofacial trauma (MFT) is a relevant problem in highly complex hospital centers. Aim: To show and update the epidemiology of the maxillofacial trauma of surgical resolution under general anesthesia, in a major operating room, treated in the Emergency Hospital Public Assistance during the period from December 2014 to December 2017 and to compare it with the epidemiological reality of other high complexity hospital centers in Chile and abroad. Materials and Method: A retrospective study was carried out and the surgical protocols of the patients operated for maxillofacial trauma, in a major operating room, under general anesthesia, in a period of 37 months, were analyzed. The variables considered were diagnosis, age, gender, facial third and affected anatomic area. Results: 127 surgical procedures were performed, for the resolution of 259 fractures. The average age of the operated patients was 33 years, with predominance of the male gender (85.8%), the lower third was the most affected (77.2%), specifically in the parasymphysis area (22.4%). Discussion: The exposed results agree with studies carried out internationally, but it has discordance in relation to the most affected anatomical area at national level comparing it with the other hospitals that analyzed its epidemiology. Conclusion: It is necessary to continue the epidemiological investigation of these pathologies and to unify diagnostic and treatment criteria in order to establish efficient prevention and treatment policies in relation to each service and jointly at the level of public health.

ABSTRACT

RESUMEN: El odontólogo juega un papel relevante en la acreditación o constatación de lesiones, especialmente si cumple labores en servicios de urgencia. Esta primera evaluación es fundamental para que el perito en lesionología forense del Servicio Médico Legal (SML) se pronuncie posteriormente respecto del pronóstico médico legal de las lesiones, señalando el tiempo que demorarán en sanar o la incapacidad laboral que generan, además de vincularlas causalmente con los hechos delictivos investigados. Se presenta información actualizada sobre el proceso de constatación de lesiones, el informe de lesiones en la legislación chilena y la participación que le cabe al cirujano dentista. La preparación de un correcto informe médicolegal de lesiones es un acto médico que no se puede delegar a otro miembro del personal de salud que no sea el profesional médico tratante. Todo cirujano dentista debe tener la capacidad de generar un primer informe de lesiones apropiado a las exigencias legales y éticas que la sociedad exige. La ley expresa la posibilidad de citar a un juicio oral, en calidad de testigo, al profesional que haya confeccionado un informe médicolegal de lesiones (constatación de lesiones), con el propósito de dar testimonio de manera directa, explicando desde un punto de vista profesional lo registrado.


ABSTRACT: The general dentist plays a major role in ascertaining and verifying injuries, especially if he or she, works in an emergency department. The initial evaluation is fundamental for the forensic dentist in order to subsequently state a prognosis of the injuries sustained, indicating how long they will take to heal, and the disability related to time away from employment. Further, it is the forensic dentist´s responsibility to evaluate and relate cause of injuries with the criminal offenses being investigated. In this study, updated information about the process is presented of medical injury verification, the injuries report in the Chilean legislation, and the participation of the dentist. The correct preparation of a forensic report of injuries is a medical responsibility that cannot be delegated to another member of the healthcare staff. All dentists must have the ability to generate a first report of injuries, according to society's legal and ethical requirements. The law states that the professional who completes a forensic report of injuries, can be summoned to trial as a witness, with the purpose of providing direct testimony, and objectively explain what he/she observed in that specific case.

Resumo Em países desenvolvidos, a violência é a principal causa de lesões bucomaxilofaciais (LBMF). No Brasil, há poucas evidências sobre tais lesões. Objetivou-se caracterizar as LBMF resultantes de agressão física em uma capital do nordeste brasileiro e analisar diferenças entre gêneros. Foram investigados 15.847 laudos do Instituto Médico Legal de São Luís/MA, ocorridos em 2012. Coletaram-se dados socioeconômicos, demográficos e características das LBMF. Utilizaram-se os testes Qui-quadrado e Exato de Fisher para avaliar diferenças entre gêneros. Dos casos periciados, 1977 eram LBMF. Vítimas do sexo feminino, com 20-59 anos de idade, cor parda, sem companheiro e empregadas foram mais afetadas. Lesões do tipo equimose, nas regiões bucinadora e labial, decorrentes do uso de instrumentos contundentes, ocasionando debilidade funcional permanente foram mais incidentes em mulheres (P < 0,05), ao passo que fratura dental, ferida contusa e perfurocontusa, decorrentes de instrumentos cortantes e perfurocontundentes, ocasionando deformidade permanente, incapacidade ocupacional e perigo de vida, em homens (P < 0,05). Conclui-se que a incidência de LBMF decorrente de agressão física é alta em São Luís, MA, e, embora as mulheres sejam mais acometidas, as LBMF em homens são mais severas.


Abstract In developed countries, violence is the main cause of oral maxillofacial (OMF) injuries. In Brazil, there are scant records of such lesions. The scope of this study was to detail the OMF injuries resulting from physical aggression in a capital of the Brazilian northeast and analyze gender differences. The Forensic Medicine Institute of São Luís, state of Maranhão, featured 15,847 reports, which occurred in 2012, and they were investigated. Socioeconomic, demographic data and OMF characteristics were analyzed. The Chi-square and Fisher's Exact tests were applied to assess gender differences. Of the cases examined, 1977 were OMF. Female victims, aged 20-59, dark-skinned, without a partner and maids were the most affected. Ecchymosis-type lesions, in the mouth and lip regions arising from the use of blunt instruments, causing permanent functional impairment, were the most prevalent in women (P < 0.05), whereas dental fracture and contusion arising from sharp instruments causing permanent deformity, occupational disability, and threat to life were detected in men (P < 0.05). The conclusion drawn is that the incidence of OMF due to physical aggression is high in São Luís, state of Maranhão, and although women are the most affected, the OMF in men are more severe.

Introducción: El conocimiento de la causa, gravedad y distribución del trauma facial y las lesiones concomitantes puede ayudar en la optimización del tratamiento inicial. Objetivo: Caracterizar el trauma maxilofacial grave en el servicio de urgencia del Hospital Universitario General Calixto García de 2015-2016. Métodos: Se realizó un estudio descriptivo y transversal en el Hospital Universitario General Calixto García, de 2015-2016. El universo lo constituyeron los pacientes que acudieron al servicio de urgencias con diagnóstico de politrauma maxilofacial y trauma maxilofacial grave. Las variables cualitativas fueron sexo, etiología, ingestión de bebidas alcohólicas y lesiones, y la variable cuantitativa fue edad. Se confeccionó una planilla de recolección de datos y la información se introdujo en una base de datos. Se emplearon números enteros, porcentajes y tablas estadísticas. Se siguieron los principios éticos para la investigación en humanos. Resultados: El sexo masculino constituyó un 73 por ciento. El grupo de edad más representado fue el de 18-30 años (32,4 por ciento), seguido por el de 31-40 y 41-50 (21,6 por ciento y 14,9 por ciento respectivamente). En el 44,6 por ciento de los pacientes el trauma estuvo relacionado con accidentes del tránsito, y en el 32,4 por ciento con violencia interpersonal. En el 50 por ciento de los pacientes se detectó aliento etílico. Predominaron las heridas mayores de 2 cm, seguidas por las de menos de esa longitud (52,7 por ciento y 28,4 por ciento, respectivamente) y la fractura mandibular (29,7 por ciento). Conclusiones: Hubo un predominio del sexo masculino y del grupo de edad de 18-30; la principal etiología fue el accidente de tránsito, seguida de la violencia interpersonal; las lesiones de tejidos blandos más frecuentes fueron las heridas mayores de 2 cm y las de tejidos duros: fracturas mandibulares. La mitad de los pacientes habían ingerido bebidas alcohólicas(AU)


Introduction: Understanding the cause, severity and distribution of facial trauma and concomitant lesions may help in the optimization of initial treatment. Objective: To characterize severe maxillofacial trauma in the emergency department of General Calixto García University Hospital from 2015-2016. Methods: A descriptive and cross-sectional study was carried out at General Calixto García University Hospital, from 2015-2016. The study population was constituted by patients who presented to the emergency department with a diagnosis of maxillofacial polytrauma and severe maxillofacial trauma. We used the following qualitative variables: sex, etiology, drinking of alcohol and lesions. We used the quantitative variable of age. A data collection form was prepared and the information was entered into a database. Whole numbers, percentages and statistical tables were used. Ethical principles for human research were followed. Results: Males accounted for 73 percent. The most represented age group was 18-30 years (32.4 percent), followed by the 31-40 and 41-50 groups (21.6 percent and 14.9 percent, respectively). In 44.6 percent of the patients the trauma was associated to traffic accidents. In 32.4 percent of the patients, the trauma was associated to interpersonal violence. Ethyl breath was detected in 50 percent of the patients. Predominating wounds were larger than 2 cm, followed by those of less than that length (52.7 percent and 28.4 percent, respectively) and mandibular fracture (29.7 percent). Conclusions: There was a predominance of males and the age group of 18-30 years. The main etiology was traffic accident, followed by interpersonal violence. The most frequent soft-tissue lesions were wounds greater than 2 cm. The most frequent hard-tissue lesions were mandibular fractures. Half of the patients had consumed alcohol(AU)

Introduction: Despite the vast literature reporting the prevalence of oral and maxillofacial diseases in the last decades, few studies have focused on lesions biopsied in the pediatric population. Objective: To determine the prevalence of oral and maxillofacial lesions that were biopsied in children and adolescents aged 0-19 years. Methods: Observational and descriptive study. We carried out a retrospective review of 862 reports of pathological examinations performed in an oral pathology laboratory of the northeast of Brazil, during the period from March 2001 to December 2009. The categories were neoplasms, hyperplastic/reactionary lesions, salivary gland lesions, bone lesions, healthy tissues and teeth, oral mucosal lesions, cystic lesions, odontogenic tumors, periapical inflammation, dental alteration and conclusive diagnosis. Results: The epidemiological profile of patients was characterized by females (53.24 percent), Caucasian (45.12 percent), with a mean age of 13.06 years. Salivary gland lesions were the category with the largest number of cases (182), and mucocele was the most prevalent histopathological diagnosis (18.44 percent), with an average size of 1.97 cm. Most cases were asymptomatic (70.88 percent). Conclusions: This study showed a predominance of lesions diagnosed as benign, the most prevalent lesions were associated with the salivary gland. Introducción: A pesar de la gran cantidad de literatura que informa sobre la prevalencia de enfermedades bucales y maxilofaciales en las últimas décadas, pocos estudios se concentraron en las lesiones biopsiadas en la población pediátrica. Objetivo: Determinar la prevalencia de las lesiones bucales y maxilofaciales que fueron biopsiadas en los niños y adolescentes de 0 a 19 años. Métodos: Estudio observacional y descriptivo. Se realizó una revisión retrospectiva de 862 informes de los exámenes patológicos realizados en un laboratorio de Patología Oral del nordeste de Brasil, durante el período comprendido entre marzo de 2001 y diciembre de 2009. Las categorías fueron: neoplasias, hiperplásicas / lesiones reactivas, lesiones de las glándulas salivales, lesiones óseas, tejidos y dientes sanos, lesiones de la mucosa bucal, lesiones quísticas, tumores odontogénicos, inflamación periapical, alteración dental y diagnóstico concluyente. Resultados: El perfil epidemiológico de los pacientes se caracterizó por las mujeres (53,24 por ciento), de raza caucásica (45,12 por ciento) con una edad media de 13,06 años. Lesiones de las glándulas salivales fueron la categoría con el mayor número de casos (182), y el mucocele fue el diagnóstico histopatológico más frecuente (18,44 por ciento), con un tamaño medio de 1,97 cm. La mayoría de los casos fueron asintomáticos (70,88 por ciento). Conclusiones: Este estudio mostró un predominio de las lesiones diagnosticadas como benignas. Las lesiones más frecuentes se relacionaron con la glándula salival. Las mujeres fueron las más afectadas(AU)

O traumatismo maxilofacial de mulheres causado porviolência tem sido mais abordado nos últimos anos. Oaumento dos casos tem sido acompanhado do aumentodas denúncias de violência contra a mulher, tornando oproblema mais visível à sociedade. Objetivo: o presentetrabalho teve o objetivo de fazer uma revisão de literaturaque aborda a prevalência de traumas maxilofaciaisdecorrentes de violência física contra mulheres e os fatoresrelacionados às lesões. Revisão de literatura: os estudosreportaram prevalências entre 24,4% e 81,0% detraumatismos maxilofaciais em mulheres causados porviolência. A maioria aponta o marido ou o companheirocomo principal agressor e o uso de força física comoprincipal tipo de agressão. Mulheres com idades entre 20e 39 anos, dependentes econômicas ou de baixa renda ecom nível de escolaridade inferior são mais susceptíveisa apresentar traumatismo maxilofacial mais prevalentenos tecidos moles. As consequências mais relatadasentre as mulheres com traumatismos maxilofaciais porviolência incluem problemas de autopercepção, de inter-relação social e de baixa autoestima. Consideraçõesfinais: conclui-se que a prevalência de traumatismosmaxilofaciais por violência em mulheres é alta, estandorelacionada à idade, à dependência econômica, ao graude escolaridade, à ocupação e a problemas psicoafetivosque podem também influenciar a dinâmica familiar. Maxillofacial trauma caused by violence to women has been addressed more in recent years. The increase of trauma has been accompanied by an increase in the reported cases of violence to women, making the problem more visible to society. Objective: by means of a literature review, the present study aimed to review the prevalence of maxillofacial trauma caused by physical violence to women and the factors related to such injuries. Literature review: the studies reported that the prevalence of maxillofacial trauma in women due to violence was between 24.4% and 81.0%. Most studies indicate the husband or partner as the primary aggressor, and the use of physical force as the main type of aggression. Women aged between 20 and 39 years, who are economic dependent or have low income and low level of education are more likely to present maxillofacial trauma more prevalent in soft tissues. The consequences most reported among women with maxillofacial trauma caused by violence included issues regarding self-perception, social interrelation, and low self-esteem. Final considerations: it is concluded that the prevalence maxillofacial trauma caused by violence to women is high and it is related to age, economic dependence, level of education, occupation, and psycho-affective problems, which may also influence family dynamics. Introducción: La región maxilofacial es vulnerable a lesiones. En Cuba son escasos los estudios realizados sobre la atención en la urgencia dentro de la traumatología maxilofacial, por lo que surgió la motivación para realizar esta investigación. Objetivo: Caracterizar desde el punto de vista clínico-epidemiológico el trauma maxilofacial en el Servicio de Urgencias del Hospital General Calixto García en el período de junio 2016 a junio 2017. Material y Métodos: Se realizó un estudio descriptivo de corte transversal del universo de pacientes (2412 pacientes) atendidos en el Servicio de Urgencias del Hospital Universitario General Calixto García, desde junio del 2016 a junio del 2017; que tuvieron como diagnóstico trauma maxilofacial. Las variables empleadas fueron: grupos de edad, sexo, factor etiológico, tipo de lesión, conducta terapéutica. Resultados: En el estudio los pacientes con edades comprendidas entre 19 y 30 años representaron 41,0 por ciento; el sexo masculino 69,0 por ciento y los accidentes de tránsito 34,6 por ciento. Las lesiones de tejido duro más frecuentes fueron las fracturas nasales (13,9 por ciento); y de tejidos blandos las heridas puramente tegumentarias (82,1 por ciento). La colocación de vendajes se utilizó en 89,1 por ciento, y la sutura de heridas en 84,1 por ciento. Conclusiones: El comportamiento del trauma maxilofacial en el Servicio de Urgencias del Hospital Universitario General Calixto García, muestra que es predominante en hombres de edad intermedia y debido a accidentes de tránsito. Hay superioridad de lesiones de tejido blando y dentro de estas, de heridas puramente tegumentarias. La conducta conservadora, en su modalidad de colocación de vendaje, es la más empleada(AU)


Introduction: The maxillofacial region is vulnerable to injuries. In Cuba, few studies related to urgent care services have been carried out in maxillofacial traumatology, which led to an increase of motivation to conduct this research. Objective: To characterize maxillofacial trauma from a clinical and epidemiological point of view in patients treated in General Calixto García Hospital from June 2016 to June 2017. Material and Methods: A descriptive cross-sectional study was conducted with the universe of patients (2412 ones) that were treated in the Emergency Service of General Calixto García University Hospital with the diagnosis of maxillofacial trauma from June 2016 to June 2017. The variables used were: age groups, sex, etiological factor, type of injury, and therapeutic behavior. Results: In the study, the patients aged between 19 and 30 years represented 41.0 percent; the male sex 69.0 percent; and the traffic accidents 34.6 percent. The most frequent hard tissue injuries were nasal fractures (13.9 percent); and the soft tissue injuries were purely integumentary wounds (82.1 percent). Bandage placement was used in 89.1 percent, and wound suture in 84.1 percent. Conclusions: The behavior of the maxillofacial trauma in the Emergency Service of General Calixto García University Hospital, showed that it predominates in men of middle age due to traffic accidents. Soft tissue injuries were the predominant type of injuries, among them, the purely integumentary wounds. The conservative behavior in its modality of bandage placement was the most commonly used(AU)

Introducción: el trauma maxilofacial es uno de los mayores retos para los servicios públicos de salud por su alta incidencia. Los protocolos mejoran la calidad de la atención y reducen la variación de la práctica clínica, por lo que es vital contar con una protocolización de alta calidad en esta área. Objetivo: evaluar protocolos de atención en la urgencia de pacientes politraumatizados, politraumatizados maxilofaciales y trauma maxilofacial grave. Métodos: se realizó un estudio descriptivo, transversal con enfoques cualitativos de mayo a junio de 2015. Se efectuó una búsqueda en Internet de protocolos de atención de urgencia del politraumatizado, politraumatizado maxilofacial y trauma maxilofacial grave en español, inglés o portugués; y nacionalmente por correo electrónico y teléfono. Se creó un instrumento evaluativo y un grupo evaluador. Se trabajó con números enteros, índices y medias. Resultados: el Protocolo de tratamiento inicial del paciente politraumático obtuvo 275 puntos, seguido por el Soporte Vital Avanzado de Trauma con 229 (índice de dificultad total de 0,93 y 0,70, respectivamente). El protocolo extranjero maxilofacial obtuvo 150 (índice de dificultad total 0,46). El ítem dos alcanzó 128 puntos (índice de dificultad total 1). El protocolo del Hospital Calixto García obtuvo 223 (índice de dificultad total 0,73) y los de Matanzas y Villa Clara 120 cada uno (índice de dificultad total 0,32). El ítem de mayor puntuación fue el cuatro con 74 puntos (índice de dificultad total 0,54). Conclusiones: los protocolos extranjeros de mayor calidad fueron Tratamiento inicial del paciente politraumático y Soporte Vital Avanzado de Trauma y el ítem mejor elaborado fue el dos. Los mejores protocolos nacionales fueron Atención al trauma grave del Hospital Calixto García y el de las provincias Villa Clara y Matanzas, y el ítem de mejor elaboración fue el cuatro(AU)


Introduction: due to its high incidence, maxillofacial trauma is one of the greatest challenges for public health services. Protocols improve the quality of care and reduce variation in clinical practice, hence the crucial importance of high-quality protocolization in the area of trauma care. Objective: evaluate the care protocols followed in the emergency management of polytrauma, maxillofacial polytrauma and severe maxillofacial trauma. Methods: a descriptive cross-sectional qualitative study was conducted from May to June 2015. An online search was carried out for protocols for the emergency management of polytrauma, maxillofacial polytrauma and severe maxillofacial trauma in Spanish, English and Portuguese. A parallel national search was performed by telephone and electronic mail. An evaluation tool was developed and an evaluation team was formed. Estimations were based on integers, indices and means. Results: the Protocol for Initial Management of Polytrauma scored 275 points, followed by Advanced Trauma Life Support with 229 (total difficulty index 0.93 and 0.70, respectively). The foreign maxillofacial protocol scored 150 points (total difficulty index 0.46). Item 2 scored 128 points (total difficulty index 1). The protocol from Calixto García Hospital scored 223 points (total difficulty index 0.73), and the ones from Matanzas and Villa Clara scored 120 each (total difficulty index 0.32). Item 4 had the highest score, with 74 points (total difficulty index 0.54). Conclusions: the highest quality foreign protocols were Initial Management of Polytrauma and Advanced Trauma Life Support, and the best developed item was number 2. The best national protocols were Management of Severe Trauma, from Calixto García Hospital, and the ones from the provinces of Villa Clara and Matanzas. The best developed item was number 4(AU)

ABSTRACT

Se realizó un estudio descriptivo y transversal de 9 932 pacientes, quienes presentaron urgencias bucofaciales, atendidos en el Cuerpo de Guardia de Cirugía Maxilofacial del Hospital Provincial Docente Clinicoquirúrgico Saturnino Lora Torres de Santiago de Cuba, desde enero del 2006 hasta diciembre del 2010, a fin de caracterizarles desde los puntos de vista clínico y terapéutico. Se halló una elevada incidencia de afectados con procesos patológicos agudos que constituyeron urgencias en el Servicio de Cirugía Maxilofacial, aunque solo 3,3 por ciento de ellos fueron hospitalizados. Hubo un predominio de pacientes con traumatismos de partes blandas, procesos inflamatorios sépticos agudos y procesos dolorosos de cabeza y cuello. La mayoría de los procederes quirúrgicos y ortopédicos se efectuaron en el cuerpo de guardia antes citado, lo cual pertenece a la categoría de cirugía menor y bajo régimen de atención ambulatoria


A descriptive and cross-sectional study of 9 932 patients who presented oral and facial emergencies, assisted in the Maxillofacial Surgery emergency room of Saturnino Lora Torres Teaching Clinical Surgical Provincial Hospital in Santiago de Cuba, was carried out from January, 2006 to December, 2010, in order to characterize them from the clinical and therapeutical points of view. It was found a high incidence of affected patients with acute pathological processes that constituted emergencies in the Maxillofacial Surgery Service, although just 3.3 percent of them were hospitalized. There was a prevalence of patients with soft parts trauma, acute septic inflammatory processes and painful processes of head and neck. Most of the surgical and orthopedic procedures were carried out in the emergency room above mentioned, which belong to the category of minor surgery and under ambulatory care

Resumo O estudo investigou o padrão espacial dos casos de traumatismo maxilofacial decorrentes de agressão com arma de fogo em adolescentes e adultos jovens a partir do local de domicílio das vítimas e analisou comparativamente os diferenciais de renda dessas áreas. Estudo transversal com dados de vítimas atendidas em três hospitais de Belo Horizonte-MG, entre janeiro de 2008 e dezembro de 2010. Endereços foram georeferenciados por geocodificação. Tendências de aleatoriedade e densidade de pontos foram analisadas por Função K de Ripley e mapas de Kernel. A interação espacial entre os domicílios de adolescentes e adultos jovens foi verificada através da Função K12. Encontrouse registros de 218 casos de agressão com arma de fogo, com predomínio do sexo masculino (89,9%) e adultos jovens (70,6%). Os domicílios se distribuíram de forma agregada no espaço urbano com nível de confiança de 99% e níveis de agregação espacial semelhantes. Os clusters de domicílios convergiram para 7 favelas ou regiões vizinhas com população de maior renda revelando propagação de eventos. Os hotspots se concentraram em favelas com histórico de crimes ligados ao narcotráfico. A incorporação do espaço na dinâmica dos eventos mostrou que a condição econômica isoladamente não limitou a vitimização.


Abstract The study investigated the spatial pattern of cases of maxillofacial injury resulting from firearm aggression among teenagers and young adults and analyzed the comparison of income differentials in these areas based on the residence of the victims. This is a cross-sectional study with data from victims attended in three hospitals in Belo Horizonte, State of Minas Gerais, from January 2008 to December 2010. The addresses of the victims were georeferenced by geocoding. Randomness and point density trends were analyzed using Ripley's K function and Kernel maps. The spatial interaction between the homes of adolescents and young adults was verified through the K12 function. Records of 218 cases of assault with a male predominance (89.9%) and young adults (70.6%) were found. Household clusters were distributed on an aggregate basis in the urban space with a confidence level of 99% and similar spatial aggregation levels. The hotspots converged on 7 shantytowns or neighboring regions with higher income population revealing spread of events. Hotspots focused on slums with a history of crimes linked to drug trafficking. The incorporation of space in the dynamics of events showed that the economic condition in isolation did not limit victimization.

RESUMO Objetivo: estudar os dados epidemiológicos de pacientes vítimas de traumas bucomaxilofaciais atendidos em um hospital de referência da Paraíba. Métodos: estudo transversal de abordagem indutiva com procedimento estatístico comparativo e técnica de pesquisa por documentação direta em campo. O universo foi constituído dos prontuários hospitalares obtidos de janeiro de 2016 a dezembro de 2017 de pacientes atendidos pelo Serviço de Cirurgia e Traumatologia Bucomaxilofacial desse hospital. A amostra foi composta por 332 pacientes de acordo com os critérios de elegibilidade do estudo. A coleta de dados foi realizada por dois examinadores previamente calibrados e os dados foram analisados descritiva e inferencialmente. Resultados: os pacientes do sexo masculino foram mais acometidos por trauma facial (83,1%), principalmente na terceira década de vida (32,2%). Acidentes motociclísticos foram a etiologia mais comum de trauma para ambos os sexos. Em relação à estatística inferencial com margem de erro fixada em 5%, não foi observada associação significativa (p>0,05) entre os sexos e os fatores etiológicos do trauma. Os ossos do nariz (38,2%) foram os ossos mais afetados e a lesão mais frequente de partes moles foi o edema, em 50,9% dos casos. Apenas 20,8% dos pacientes com fraturas ósseas foram politraumatizados. Conclusão: as vítimas de traumatismo bucomaxilofacial atendidas em nosso hospital são predominantemente homens na terceira década de vida, envolvidos em acidentes motociclísticos e com lesões em ossos do nariz.


ABSTRACT Objective: to study the epidemiological data of patients suffering from buccomaxillofacial trauma treated at a referral hospital in the State of Paraíba. Methods: we conducted a cross-sectional study of inductive approach, with a comparative statistical procedure and research technique by field direct documentation. The sample comprised hospital records obtained from January 2016 to December 2017 of patients attended by the Service of Buccomaxillofacial Surgery and Traumatology of this hospital, and consisted of 332 patients according to the study's eligibility criteria. Two previously calibrated examiners collected and analyzed the data, both descriptively and inferentially. Results: males sustained the majority of facial trauma (83.1%), mainly in the third decade of life (32.2%). Motorcycle accidents were the most common etiology of trauma for both genders. In relation to inferential statistics with a margin of error of 5%, there was no significant association (p>0.05) between the genders and the trauma etiological factors. The bones of the nose (38.2%) were the most affected bones and the most frequent soft tissue injury was edema, in 50.9% of cases. Only 20.8% of the patients with bone fractures were polytraumatized. Conclusion: the victims of oral and maxillofacial trauma attended at our hospital are predominantly men in the third decade of life, involved in motorcycle accidents and sustaining lesions in the nose bones nose.

ABSTRACT

RESUMO O objetivo deste estudo foi determinar as circunstâncias das agressões e os padrões dos traumas maxilofaciais entre vítimas de violência interpessoal. Tratou-se de um estudo transversal e exploratório feito a partir da análise de 7.132 registros médico-legais e sociais de vítimas de violência interpessoal atendidas em um Centro de Medicina Legal e Odontologia Forense. Foi feita estatística descritiva e multivariada, usando Análise de Correspondência Múltipla. Três grupos com perfis distintos de vitimização foram identificados. O primeiro grupo foi formado majoritariamente por homens, de diferentes faixas etárias, vítimas de violência comunitária, resultando em fratura de ossos faciais ou fratura dentoalveolar. O segundo grupo foi composto essencialmente por adolescentes (10-19 anos), de ambos os sexos, vítimas de violência interpessoal e que não apresentaram um padrão específico de trauma. O terceiro grupo reuniu mulheres, adultas (≥ 20 anos), vítimas de violência doméstica, resultando em lesão de tecidos moles da face ou em outras regiões do corpo. Os resultados sugerem que as características sociodemográficas e circunstanciais são fatores importantes na vitimização por traumatismo maxilofacial e violência interpessoal.


Abstract The aim of this study was to determine the circumstances of aggressions and patterns of maxillofacial injuries among victims of interpersonal violence. This was a cross-sectional and exploratory study conducted from the analysis of 7,132 medical-legal and social records of interpersonal violence victims seen in a Forensic Medicine and Dentistry Center. Descriptive and multivariate statistics were performed using Multiple Correspondence Analysis. Three groups with different victimization profiles were identified. The first group was mainly composed of men of different age groups, victims of community violence that resulted in facial bones or dentoalveolar fracture. The second group was mainly composed of adolescents (10-19 years) of both sexes, victims of interpersonal violence and without specific pattern of injuries. The third group was composed of adult women (≥ 20 years) victims of domestic violence that resulted in injuries of soft tissues of face or other body regions. The results suggest that sociodemographic and circumstantial characteristics are important factors in victimization by maxillofacial injuries and interpersonal violence.

Objetivo: Presentar la experiencia de la especialidad en cirugía y traumatología bucal y maxilofacial en el Hospital de Urgencia Asistencia Pública, a través de la epidemiología de los tratamientos quirúrgicos realizados en pabellón central, bajo anestesia general, durante el período comprendido entre noviembre del 2014 y julio del 2016. Material y métodos: Se realizó un estudio retrospectivo en el que se analizaron los protocolos operatorios de los pacientes intervenidos durante el período. Se recopiló información de la base de datos del pabellón central sobre el diagnóstico, la edad, el sexo y el tipo de procedimiento quirúrgico. Se incluyeron 125 procedimientos quirúrgicos realizados en 109 pacientes. Resultados: La edad promedio de los pacientes intervenidos fue de 38 años, con preponderancia del sexo masculino (70,6%). El 50,4% de los procedimientos quirúrgicos correspondieron a traumatismos en el territorio maxilofacial, de los cuales la fractura mandibular fue la lesión más frecuente (68,3%). El 46,4% de los procedimientos correspondieron a infecciones en el territorio maxilofacial, en los cuales la localización más frecuente fue el espacio anatómico submandibular (25,8%). El resto de los procedimientos se relacionó con otro tipo de diagnósticos (3,2%). Discusión: En general los resultados concuerdan con estudios similares realizados en hospitales de atención de urgencia. Conclusión: Dentro de las patologías de urgencia del territorio maxilofacial, las lesiones traumáticas e infecciones son las que con mayor frecuencia requieren de cirugía mayor bajo anestesia general. Los pacientes intervenidos con cirugía bajo anestesia general en pabellón central son principalmente hombres, siendo las fracturas mandibulares y la infección del espacio submandibular las lesiones más frecuentes en cada tipo de patología.


Objective: To determine the experience of the specialty in Oral and Maxillofacial Surgery and Traumatology at the Hospital Emergency Assistance Publique, through epidemiology of surgical treatments carried out in major operating room, under general anesthesia during the period between November 2014 and July 2016. Material and methods: A retrospective study where the operative protocols of patients operated during the period analyzed was performed. Database information from major operating room for diagnosis, age, sex and type of surgical procedures was collected. 125 surgical procedures performed in 109 patients were included. Results: The average age of the operated patients was 38 years with a preponderance of males (70.6%); 50.4% of surgical procedures were for injuries in the maxillofacial area, where the mandibular fracture was the most common lesion (68.3%); 46.4% of the procedures were for infections in maxillofacial area, where the most common location was the submandibular anatomical space (25.8%). The rest of the procedures related to other diseases (3.2%). Discussion: In general the results are consistent with similar studies in emergency care hospitals. Conclusion: Within the pathologies of urgency of the maxillofacial territory, traumatic injuries and infections are those that most frequently require major surgery under general anesthesia. Patients undergoing surgery under general anesthesia in major operating room are mainly men, with mandibular fractures and submandibular space infections being the most frequent lesions in each type of pathology.

Introducción: en el trauma encontramos al huésped (paciente) y a un vector de transmisión (vehículo de motor, arma de fuego, etc.). Objetivo: realizar una revisión bibliográfica sobre aspectos generales del trauma maxilofacial, incluyendo conceptos generales, características de la región y del trauma maxilofacial, epidemiología y registro estadístico del trauma. Métodos: se realizó una revisión bibliográfica de noviembre a diciembre de 2014. Se evaluaron revistas de impacto de Web of Sciencies (40 revistas), 1 cubana y 3 libros. Se consultaron las bases de datos de MEDLINE, PubMed y ScieELO con los descriptores: trauma, maxilofacial trauma, epidemiology. Se incluyeron artículos en idioma inglés y español, preferentemente de los últimos 5 años. Se obtuvieron 132 artículos. El estudio se circunscribió a 55. Análisis e integración de la información: conceptos como el de trauma, y el abordaje de las características de la región y su traumatología, son punto de partida en este tema. Se observa una tendencia al cambio en los patrones epidemiológicos. La implementación de un registro estadístico de trauma es esencial en un sistema de atención a estos pacientes. Conclusiones: la revisión de la bibliografía permite revisar conceptos como el de trauma, que se define como la experiencia de un individuo de un evento o condición perdurable que es una amenaza real o percibida para su vida e integridad personal, o la de un cuidador o familiar. Las principales causas de este tipo de fractura mundialmente son los accidentes de tráfico, agresiones, caídas y lesiones relacionadas con el deporte. Las lesiones maxilofaciales varían de simples a complejas y pueden comprometer piel, tejidos blandos, así como huesos. Las fracturas faciales a menudo implican morbilidad grave y consecuencias ocasionalmente mortales. La implementación de un registro estadístico de trauma conlleva controlar y mejorar la calidad de los programas de atención traumatológica(AU)


Introduction: in trauma there is a host (patient) and a vector of transmission (motor vehicle, gun, etc.). Objective: to conduct a literature review on general aspects of maxillofacial trauma, including general concepts, characteristics of the region and maxillofacial trauma, epidemiology and statistical registration of trauma. Methods: a literature review was conducted in the period from November-December 2014 on Web of Sciences' impact journals (40 journals) and 1 Cuban. Three books were evaluated. Databases of MEDLINE, PubMed and SciELO with the use of descriptors: "trauma", "maxillofacial trauma", "epidemiology" were consulted. Articles in English and Spanish language, preferably publications in the last five years were included. 132 articles were obtained. The study was limited to 55. Data analysis and integration: concepts such as that of trauma, as well as addressing the characteristics of the region and its traumatology, are a starting point to this topic. We observed a tendency towards changing epidemiologic patterns. The implementation of a trauma statistic record is key to a system for the management of these patients. Conclusions: review of the literature allows reviewing concepts such as trauma, defined as an individual's experience of an event or condition that is enduring a real or perceived threat to their lives and safety, or that of a caretaker or familiar. The main causes of these fractures are world traffic accidents, assaults, falls and sports-related injuries. Maxillofacial injuries range from simple to complex and can involve skin, soft tissues and bones. Facial fractures often involve serious morbidity and occasionally fatal. The implementation of a statistical register of trauma involves controlling and improving the quality of trauma care programs(AU)

ABSTRACT

Fundamento: El conocimiento de la morbilidad de los defectos bucomaxilofaciales brinda información valiosa que permite ser utilizada en pos de garantizar una atención integral a los pacientes con estos defectos. Objetivo: Determinar la morbilidad y caracterizar los defectos bucomaxilofaciales en el Hospital General Provincial Camilo Cienfuegos. Sancti Spíritus. Metodología: Se realizó un estudio descriptivo de corte transversal, en una población constituida por 475 pacientes que recibieron tratamiento en el Departamento de Prótesis Bucomaxilofacial entre noviembre de 1998 y diciembre de 2010. Se utilizaron métodos del nivel teórico, empírico y estadístico. Resultados: Predominó el grupo de edad de 60 y más años (38,1 %) y el sexo masculino (75,4 %). Prevaleció el defecto ocular (73,9 %) y el factor etiológico traumático (56,6 %). La prótesis más requerida por años fue la ocular con 351 y el año más representativo el 2001. Conclusiones: Se observó un predominio en el grupo de edad de 60 y más años, y el sexo masculino. En cuanto a la localización el defecto ocular resultó ser el más representado, en casi las tres cuartas partes de la población, con un predominio del trauma como factor causal. El tipo de prótesis más demandada fue la ocular y su cantidad se comportó de forma irregular en los diferentes años.


Background: The knowledge of the morbidity of the buccomaxillofacial defects offers valuable information that allows to be used after guaranteeing an integral attention to the patients with these defects. Objective: To determine the morbidity and to characterize the buccomaxillofacial defects in the Provincial General Hospital Camilo Cienfuegos. Sancti Spíritus. Methodology: It was carried out a descriptive study of traverse cut, in a population constituted by 475 patients that received treatment in the Department of buccomaxillofacial Prosthesis between November 1998 and December 2010. The theoretical, empiric and statistical level methods were used. Results: Prevailed the group of age from 60 and more years old (38,1 %) and the masculine sex (75,4 %). the ocular defect prevailed (73,9 %) and the traumatic etiologic factor (56,6 %). The prosthesis more required per years was the ocular one with 351 and the most representative year was 2001. Conclusions: Was observed a prevalence in the group of age from 60 years old and more, and the masculine sex. As for the localization the ocular defect turned out to be the most represented one, in almost fourth three parts of the population , with a prevalence of the trauma like causal factor. The ocular prosthesis was most demanded and its quantity behaved in an irregular way in the different years.

ABSTRACT

The objective of this study was to determine the epidemiological profile of women victims of violence and characterize the injuries affecting their maxillofacial region. This was an analytic study using secondary data from 470 expert reports of domestic violence involving women, collected from January 2007 to December 2011 in the Department of Dentistry of the Institute of Legal Medicine in the city of Feira de Santana, Bahia, Brazil. The majority of the examined women were Afro-caucasian (72.6 %), single (74.9%), and with mean age of 30.4 years. The buccal (51 %) and orbital (8.8 %) regions were the most affected extraoral sites. As to the oral cavity, the most affected sites were teeth (50.8 %) and the lip mucosa (35 %). In most cases the lesions against women were associated with an aggressor having a close relationship with the victim. The most prevalent lesions were edema; fracture; tooth dislocation; and bruise. These findings can subsidize the development of policies to prevent violence against women, and for public safety.


El objetivo fue determinar el perfil epidemiológico de mujeres víctimas de violencia y las lesiones que les acometieron la región oral y maxilofacial. Se trata de un estudio sobre datos secundarios realizado a partir del análisis de 470 informes periciales de violencia doméstica contra mujeres, recopilados entre enero del 2007 a diciembre del 2011, en el área de Odontología del Instituto Médico Legal de Feira de Santana (Bahía/Brasil). La mayoría de las mujeres estudiadas eran afrocaucásicas (72,6 %), solteras (74,9 %), con un promedio de edad de 30,4 años. En relación a la localización extraoral, las regiones bucal (51 %) y orbital (8,8 %) fueron las más afectadas. Los dientes (50,8 %) y la mucosa labial (35 %) fueron las estructuras intraorales más afectadas. En la mayoría de los casos, las lesiones estaban asociadas a un agresor con una relación cercana a las mujeres. Las lesiones más prevalentes fueron edemas, fracturas, luxación dental y equimosis. Estas hallazgos pueden colaborar en el desarrollo de acciones para combatir la violencia contra la mujer y para la seguridad pública.

Fundamento: El aumento considerable de pacientes necesitados de tratamiento rehabilitador con lesiones bucomaxilofaciales en la provincia de Sancti Spíritus, es una de las causa importante para que se investigue este problema de salud, lo cual permitirá la planificación de los recursos para la rehabilitación de cada paciente. Objetivo: Describir las características clínico-epidemiológicas de los pacientes con lesiones bucomaxilofaciales que necesitan tratamiento rehabilitador. Metodología: Se realizó una investigación descriptiva en el período comprendido entre noviembre de 1998 y febrero del 2008, la población quedó conformada por 407 pacientes rehabilitados con prótesis restauratriz, se realizó la distribución de frecuencia según variables seleccionadas, edad, sexo, color de la piel, experiencia protésica, origen y localización del defecto. Resultados: Predominó el defecto ocular, el sexo más afectado fue el masculino; asimismo los pacientes de raza blanca fueron los que predominaron y el factor etiológico de mayor incidencia fue el traumático. Conclusiones: El comportamiento clínico-epidemiológico de las lesiones bucomaxilofacial en la población espirituana mostró una necesidad real de tratamiento rehabilitador.


Background: The considerable increase of needy patients of rehabilitative treatment with buccomaxillofacial lesions in the county of Sancti Spíritus is one of the important causes of this problem of health to be investigated, which will allow the planning of the resources for each patient's rehabilitation. Objective: To describe the clinical-epidemic behavior of the buccomaxillofacial lesions that need of rehabilitative treatment. Methodology: It was carried out a descriptive investigation in the period understood from November 1998 and February 2008, the population was formed by 407 patients rehabilitated with restorative prosthesis, it was carried out the distribution of frequency according to selected variables, age, sex, color of the skin, prosthetic experience, origin and localization of the defect. Results: The ocular defect prevailed, the most affected sex was the masculine; also the patients of white race were those that prevailed and the etiological factor of more incidences was the traumatic. Conclusions: The clinical-epidemic behavior of the buccomaxillofacial lesions in the population from Sancti Spíritus showed a real necessity of rehabilitative treatment.

ABSTRACT

A violência tem sido um tema amplamente discutido em diversos setores da sociedade, quer pelo reflexo nos indicadores de segurança pública, pela influência no cotidiano dos indivíduos ou pela presença constante de vítimas nos serviços de saúde. O estudo avaliou comparativamente as diferenças de vitimização entre os gêneros a partir dos traumatismos maxilofaciais como marcadores de violência urbana. Trata-se de um estudo transversal com dados coletados em três hospitais especializados no atendimento a politraumatismos em Belo Horizonte (MG), entre janeiro de 2008 e dezembro de 2010. As análises envolveram estatística descritiva e multivariada por regressão logística. Identificou-se o registro de 7.063 vítimas, sendo 55,1% de violência interpessoal. A maioria das vítimas era do sexo masculino (71,2%). Nos homens, as agressões por arma de fogo, arma branca e acidentes motocilísticos foram as mais importantes quando comparadas às agressões nuas ou sem uso de armas. As fraturas múltiplas foram o tipo de traumatismo que melhor caracterizou o perfil de vitimização para o sexo masculino comparativamente às lesões de partes moles. O gênero é um importante fator na vitimização por traumatismo maxilofacial e violência urbana, sendo que os homens são as principais vítimas. .


Urban violence is a widely discussed topic in various sectors of society, either due to its impact on public health indicators and its influence on the everyday life of individuals or the constant presence of casualties in the health services. This study compares differences in victimization between the genders based on maxillofacial injuries as markers of urban violence. This is a cross-sectional study with data collected in three hospitals of reference for multiple traumatic injuries in Belo Horizonte in the state of Minas Gerais, between January 2008 and December 2010. The analysis included descriptive and multivariate statistics using logistic regression. There were records of 7,063 victims, 55.1% of which involved interpersonal violence. The majority of victims were males (71.2%). Among the male victims, firearm and knife-inflicted aggression and motorcycle accidents were more frequent than aggression without the use of a weapon. Multiple fractures were the type of injury that best characterized the profile of victimization among males compared to soft tissue injuries. Gender is an important factor in victimization resulting in maxillofacial injuries and urban violence, in which males are the main victims.
